What the evidence says:
The 药典 indication for 罗汉果 is lung-heat/dry cough, sore throat, hoarseness and dry constipation — established traditional use, not trial evidence. Its practical advantage is real though: it sweetens without sugar, so it makes a soothing warm drink that a diabetic can also use.

How to make it

Break open a quarter to half a dried fruit, simmer in 500 ml water 10–15 min, strain. Sip warm through the day. Naturally intensely sweet — do not add sugar.

Why it works

Mogrosides give the sweetness with negligible calories and no glycaemic load, which is why it suits diabetics as a honey alternative.

罗汉果 is a Qing-dynasty addition — NOT in the Shennong Bencao Jing, despite how ancient the drink feels.

Cautions: Cooling — go easy if you run cold or have loose stools. Same rule as above: cough beyond 3 weeks, or with blood or breathlessness, needs assessment.
